Proper identification is required from everyone. You must be eighteen years of age to be pierced without the consent of a parent or legal guardian. Proper identification must have the piercee's name, birth date, and photo.
 

Forms of Proper Identification

-Driver's License

-State issued Identification Card

-Birth certificate with photo-identification (full name and photo)

The identification will be photocopied on the back of the release form. The release form is filled out and signed by the client.
 

Requirements for Piercing Minors

-Parent or legal guardian must be present

-Valid parent/legal guardian identification is required

-Legal guardian must have legal documentation of guardianship

-Valid minor identification is required

Both forms of identification will be photocopied on the back of the release form. Also, the legal guardianship documents will be photocopied and attached to the release form. All personal information is filled out by the minor. The parent/legal guardian and the minor both sign the release form. *Stepparents, grandparents, siblings, aunts, uncles, or friends cannot sign for minors without legal guardianship documentation.
